STUDIO COMPARATIVO DELL'EFFICACIA DI OLIO OZONIZZATO NELLE GENGIVOSTOMATITI RISPETTO AI TRATTAMENTI TRADIZIONALI 
presentato al congresso di parodontologia presso il Collegio dei Docenti a Roma il 5/8 aprile 2006.
Uno studio su 160 bambini affetti da gengivostomatite aftosa di età compresa tra 0 e 15 anni è stato condotto usando olio ozonizzato micronizzato nel cavo orale. La sintomatologia comprendeva febbre, anoressia, ipersalivazione, dolore gengivale, astenia e irrequietezza durante la fase evolutiva della malattia. Un gruppo di 60 bambini è stato trattato giornalmente con olio ozonizzato mentre i restanti sono stati suddivisi in tre gruppi controllo trattati con diverse sostanze disinfettanti del cavo orale. Nel gruppo trattato con olio ozonizzato si è ottenuta la guarigione e la scomparsa delle lesioni nel 75% dei casi tra il 3° e il 7° giorno di trattamento dimostrando una significativa efficacia rispetto agli altri gruppi controllo. La caratteristica germicida dell'olio ozonizzato e l'ottima tollerabilità indica questo agente quale il più indicato nei casi di gengivostomatite sia in bambini che in adulti

La perdita di diversi elementi dentari rende prioritaria un’analisi dei fattori causali che deve rigorosamente precedere qualsiasi programmazione implanto protesica.
Nell’ambito di una valutazione gnatologica completa, un rilievo cefalo metrico, unitamente a quello condilografico, consente l’ interpretazione di fattori causali di danno pregresso, nonchè la programmazione dei possibili interventi migliorativi.
Tutti i casi oggetto di estesa riabilitazione implanto protesica vengono sottoposti allo stesso protocollo dove, oltre agli altri parametri, viene valutata la altezza facciale inferiore, la dimensione verticale dell’occlusione, l’inclinazione del piano occlusale e l’angolo di disclusione tra gli elementi dentari. Viene eseguita una ceratura diagnostica dopo
 aver programmato il rapporto protesico ideale tra le arcate. Sulla ceratura viene realizzata una mascherina chirurgica per una valutazione radiologica preventiva e per il posizionamento degli impianti.
Gli esempi riportati rappresentano un caso di edentulia totale inferiore con perdita di dimensione verticale, trattata mediante implantologia con carico immediato, dove è auspicabile la riduzione del trauma occlusale su impianti non ancora integrati.
Altri casi con perdita di supporto occlusale posteriore e piani occlusali non compatibili con un corretto angolo di disclusione.
Questi principi di occlusione, fondati su elementi di biomeccanica applicata, ottenuti da uno studio affatto invasivo
, proteggeranno le successive ricostruzioni protesiche e le sottostanti fixtures implantari dai traumi determinati da una biomeccanica sfavorevole.
Non tutti i parametri possono essere modificati protesicamente, la loro conoscenza consente tuttavia di comprendere i limiti degli interventi che possono essere eseguiti e, nel caso, concordare con il paziente eventuali soluzioni alternative.
La ceratura diagnostica preventiva, non realizzata unicamente in funzione della distribuzione degli spazi protesici, quanto per modificare i patterns funzionali del sistema, costituisce uno strumento indispensabile al chirurgo, oltre al protesista ed eventualmente all’ortodonzista, con la prospettiva di migliorare la funzionalità dell’apparato masticatorio e
 del sistema cranio cervico mandibolare nell’ambito di una riabilitazione funzionale complessa

1 ) B A C K G R O U N D
Research in surface and connection technology in implantology has brought clinical success, saving both
the bone level and predictability of the integration of dental implants. However, prosthetic programming is
often based on an empiric evaluation where the prosthetic space distribution is not backed by a
functional analysis. Radiographs do not give enough information for the comprehensive analysis involved
in rehabilitation. In a complete diagnostic evaluation, cephalometric and condylographic analysis are
needed to interpret the determinant factors of the damage, as well as to see the new arches and occlusal
relation that will reduce the risk factors for implant integration and prosthetic functional overload.

2 ) M A T E R I A L S  A N D  M E T H O D S
All cases that need rehabilitation follow the same protocol. In addition, other parameters such as the
height of the lower face, the vertical dimension of occlusion, the inclination of the occlusal plane and the
teeth disocclusion angle are evaluated. Functional analysis includes:

Condylographic examination to document the tmj dynamics together with the inclination of the
condylar eminences.

Cephalometric analysis to measure the height of the lower face and the inclination of the occlusal
plane, to clarify the correlation between the inclination of the anterior functional guidance and the
inclination of the temporo-mandibular joints as recorded by condylography. This correlation will give
the disocclusion angle of the cusps which is an essential parameter in diagnosis and will further aid the dental technician.

Clinical evaluation of the ligaments and muscles of the cranial, cervical and mandibular
regions in order to identify the muscular vectors and the compensation mechanism.

Study casts of dental arches mounted on an individual articulator (reference sl), registered on
patient’s data from the condylography record for, functional extraoral analysis of the masticatory
system.

Diagnostic wax-up after programming the ideal prosthetic treatment report.
The diagnostic wax-up serves for a functional verification of the efficacy of the therapeutic
hypothesis. It expresses the symbiosis between clinical analysis and the rehabilitation
plan, where the clinician and the technician examine the final aim of the implant, before
actually starting, as no further orthodontic work can be done later.

3 ) R E S U L T S
This information helps to analyze the c aus es of the malocc lus ion, and to modify the bio
mec hanics of the s ys tem: c ephalometr y analyzes and  modifies tr ough the Vis ual
Tr eatment Objectives the ver tic al dimens ion of occ lus ion and evaluates the c ompensation
mec hanism of maloc c lus ion. The mas tic ator y effic ienc y and inter fer ences in functional
movement will be gover ned by the teeth dis occ lus ion angle, as well as the r elative
occ lus al plane inc lination.

4 ) D I S C U S S I O N
Today’s high level of technology in implantology especially in surface coatings, and guided surgery for
predictable success, lacks the same level of evidence- based emphasis on analysis of the biomechanics
of the system where the implants will be placed.
Occlusion principles enable to plan the ideal fixture positioning in a functional relation in order to avoid
trauma due to unfavorable biomechanics.
The diagnostic waxing-up is done not only for the prosthetic space distribution, but to modify the
functional patterns of the masticatory apparatus. It is a useful tool for the surgeon and for all the other
specialists involved, to improve the masticatory organ and cranio-cervico-mandibular system function in a
comprehensive full-mouth rehabilitation
